Supported Community Living Service
ChildServe Supported Community Living
 
ChildServe's Supported Community Living service provides individualized skill-building services and support to children and families in their community. The focus of the service is to work on skill development that ultimately leads to independence. We work on outcome-based goals that will improve children's living skills.
 
 
  • SCL staff support skill-building and learning using activities your child enjoys and wants to do. By supporting your child’s self-directed learning, no one will do for your child what your child can accomplish on his or her own.
  • Living and learning in your child’s community is an essential part of the SCL program. Even if you think there are no opportunities near your home, we can help you find them
  • Improved communication, motor skills, and social skills are only a few of the goals covered in the SCL experience. Specific goal-setting combines your child’s life skill needs and personal dreams to create opportunities for independent living.

Individual services
ChildServe provides individual supported community living services. Each child is assigned a staff person who will work with them on individual goals in the child's home or other community-based settings.

SCL OPPORTUNITIES

Below are some ideas to help you identify opportunities well-suited for SCL:

Daily Living Skills

  • making a bed
  • cooking a family meal
  • cleaning his or her room
  • learning laundry skills

Individual Advocacy

  • registering to vote
  • obtaining and using a library card
  • understanding his or her rights

Community Participation

  • developing a plan for volunteering
  • participating in scouting or other youth/civic groups
  • participating in a Parks and Recreation program
  • learning skills to keep themselves and others safe

Socialization or Communication Goals

  • taking classes at the YMCA
  • planning a night out with friend
  • attending a school dance or social function
  • after-school activities

ELIGIBILITY
Your child must be eligible for BI (Brain Injury) or ID (Intellectual Disability) Home and Community Based Waiver Service. SCL services are based on your child’s needs. Your Case Manager can assist you in making a referral.
