ChildServe offers the following community services:
ChildServe Case Management provides and promotes comprehensive, family-centered services and supports for children and young adults with disabilities living at home or in the community. ChildServe case managers provide advocacy for individuals and are knowledgeable about many services and funding options for children and families. The Case Manager serves as the primary contact for all service coordination activities.
ChildServe Childcare is now enrolling children ages 6 weeks through Kindergarten! Our degree-certified and tenured teachers establish goals and objectives for each child to set the stage for success in Kindergarten and beyond. Our nurturing, child-centered environment encourages choice and diversity.
ChildServe Day Habilitation services assist or support individuals in developing or maintaining life skills and community integration. Programming is coordinated on-site and in the community to enhance opportunities to learn in a variety of environments. Day Habilitation services are designed to assist teens and young adults to successfully transition to adulthood by increasing community integration and life skills.
ChildServe Day Health program offers integrated therapy, nursing, early intervention, skill development, and more for Medicaid-eligible children with special health care needs. Along with creating individual care plans for each child, we work with family members to provide education and support. Our on-site pediatric speech, physical, and occupational therapy is available to eligible children, and trained staff provide personal care assistance throughout the day. Breakfast, lunch, and snacks are also provided as part of the Day Health program.
ChildServe Homecare's goal is to promote independence and a higher quality of life by empowering our patients and their families to be actively involved in their care. We work closely with each child’s family, physicians, other health care professionals, and service providers to develop a child-centered, family-focused plan of care. Our rigorously-trained, credentialed staff ensure each family’s homecare services are provided with respect, dignity, and compassion.
ChildServe Respite is designed to give families a break from the day-to-day care involved in supporting a child with special health care needs. We offer individual and specialized respite, where children spend time one-on-one with a caregiver in the family’s home or community. Group respite options, like sleepovers, Kids Club, and Camp ChildServe, offer great opportunities for socialization and fun!
ChildServe Supported Community Living service provides individualized skill-building services and support to children and families in their community. The focus of the service is to work on skill development that ultimately leads to independence. We work on outcome-based goals that will improve children's living skills.
