Child
Serve's Supported Community Living service provides individualized skill-building services and support to children and families in their community. The focus of the service is to work on skill development that ultimately leads to independence. We work on outcome-based goals that will improve children's living skills.
Goals
Each child's plan includes his/her own specific skills and goals, which may include:
- Personal cares
- Home skills
- Communication
- Accessing community resources
- Self advocacy
- Safety
- Socialization
- Transportation, etc.
Individual services
Child
Serve provides individual supported community living services. Each child is assigned a staff person who will work with them on individual goals in the child's home or other community-based settings.
Community outings include trips to:
- Parks
- Restaurants
- Public centers
- Bowling alleys
- Swimming pools
- Living skills centers
